Request an address assignment

Use this form to submit an address request for a property within the City of Louisville. Address requests may include assigning a new address, changing an existing address, or correcting address information. Please complete all required fields and upload any supporting documentation to assist with the review process. Community Development staff will review your request and contact you if additional information is needed. Requests are typically processed within 2 business days but may take up to 5 business days, depending on the complexity of the request.
